EMS-optimized ECG for rate and rhythm monitoring
The EXG™ 4-Electrode Wearable ECG — also referred to as the 4-Lead — is designed with EMS crews in mind. It's the EMS-optimized 4-Electrode configuration of the EXG™ platform, built for rate and rhythm monitoring on lower-acuity patients where full diagnostic 12-Lead information may not be required, and where fast, flexible electrode placement matters more than lead count.
Now available through Heart Medical for U.S. EMS agencies.
Why the 4-Electrode configuration matters for EMS
Prehospital cardiac assessment isn't always about STEMI transport. A large share of EMS runs involve stable patients who need rate and rhythm monitoring rather than full diagnostic ECG — post-syncope workups, medication assessments, complaints of palpitations, and countless other lower-acuity encounters where a fast, defensible tracing is what the crew actually needs.
The EXG™ 4-Electrode (4-Lead) is optimized for exactly those situations:
- Rapid deployment — single connection enables setup in under 60 seconds
- Multiple placement options — wrist-to-wrist, limb-to-limb, or limb-to-torso configurations for whatever the patient position allows
- Single-cable design — one universal adapter compatible with all portable ECG monitors; same adapter as the EXG™ 12-Lead flagship
For high-acuity STEMI transport, EMS agencies typically carry the EXG™ 12-Lead flagship alongside this 4-Electrode — 12-Lead for STEMI workup, 4-Electrode (4-Lead) for the majority of everyday runs.
Key features — 4-Electrode (4-Lead) configuration
- Rapid deployment — single connection enables setup in under 60 seconds; one universal adapter compatible with all portable ECG monitors
- Rate and rhythm monitoring for lower-acuity patients
-
Multiple placement configurations:
- Wrist-to-wrist
- Limb-to-limb
- Limb-to-torso
- Motion artifact reduction — improved methods for high-activity environments
- Single-cable design — uses the same adapter as EXG™ 12-Lead continuous wearable patch
- Disposable, per-patient — single-use consumable
- Universal compatibility — one cable fits any standard ECG monitor; works with LP15, X Series, and other EMS ECG monitors
Intended use
The EXG™ 4-Electrode (4-Lead) is intended for assessing lower-acuity patients where you may not require full diagnostic information. For diagnostic 12-Lead ECG in STEMI transport or hospital cardiology workflows, use the EXG™ 12-Lead flagship instead.
Clinical applications
- EMS low-acuity encounters — post-syncope, palpitations, medication assessments, general rate & rhythm documentation
- Field triage where full 12-lead may not be needed
- Interfacility transports for stable patients
- Non-STEMI cardiac complaint workups on scene or during transport
- Any prehospital situation where fast electrode placement and multiple placement options matter more than lead count
Certifications and compliance
Certified for professional medical use: FDA-Registered, AHA-Compliant & ISO-13485 Certified.

Explore other configurations
The 4-Electrode (4-Lead) is the EMS-optimized configuration of the EXG™ platform. For diagnostic ECG or specialty views:
- EXG™ 12-Lead (Flagship) — high-acuity STEMI transport and hospital diagnostic ECG; the flagship
- EXG™ 5-Lead — hospital continuous rhythm monitoring
- EXG™ 3-Lead (Reversible) — Right-sided + Posterior visualization for confirmatory MI workup

Purchase and configuration workflow
- Request a scoped quote. Share your agency's monthly EMS run volume, station count, and existing ECG monitor infrastructure.
- Configuration walkthrough. Most EMS agencies carry both 4-Lead (for low-acuity) and 12-Lead (for STEMI transport). Confirm the volume mix.
- Deployment planning. Coordinate with your biomed / clinical engineering team on integration and applications training.
- Standing-order supply. EXG™ 4-Lead is disposable per-patient — predictable EMS run volumes fit well with standing-order setup.
